DECLARATION

I am interested in participating in the Melbourne Forum. I understand that membership is not automatic and that this application for membership is subject to committee approval. I also understand that the current membership fee of $594 (includes GST) is payable on being invited to join and then each year on my anniversary date to remain a member of Melbourne Forum.

I acknowledge the core principles of the Melbourne Forum are that:

1. It provides a welcoming and open environment for the discussion of social, economic, political and public issues of relevance Melbourne and Australia, and to our role in the world.
